Discharge Process
Seamless procedures to complete your hospital discharge and prepare you for a safe return home.
Before Discharge
According to FV Hospital’s policy, your doctor will determine when you are ready for discharge and will inform you at least one day in advance. To ensure a smooth discharge process, the following steps will be completed before you leave the hospital:

Your doctor will review your medical records, assess your treatment progress, prescribe any necessary medications and arrange follow-up appointments if required.

A medical secretary will review your prescription, forward it to the Pharmacy Department for preparation, and provide you with an itemised hospital bill.

A member of the Patient Experience Department will visit you to gather feedback and discuss your experience of the care and services provided at FV Hospital.
Checkout & Payment
Discharge Time Policy
After discharge procedures are completed, you will be guided through payment and discharge. The official check-out time at FV Hospital is before 11:00 am. Late check-out may incur additional room charges.
Hospital Bill Payment
To make the discharge process as comfortable and convenient as possible, FV Hospital allows payment to be completed directly in the patient’s room.
Receiving Medication
Your prescription is sent electronically to the Pharmacy Department, where pharmacists carefully review and prepare your medications before delivering them directly to your room. They will provide personalised counselling on how to take your medicines at home, including dosage, timing, administration instructions, and any important precautions regarding possible side effects or drug interactions.
Documents Received upon Discharge

Discharge prescription.

Discharge summary (signed by doctor).

Laboratory results (including blood type card).

Imaging results (film or CD, if applicable).

Procedure results (Cathlab, ECG, echocardiogram, endoscopy, stress test, etc,)

Surgical report (if applicable).

Rehabilitation indication.

Patient instruction documents.

Discharge certificate or referral letter.

Birth certificate or death certificate (if applicable).

Follow-up appointment card.

Child health book (for newborns).

Fit-to-fly certificate (if required).
