Procedure for Changing Primary Healthcare Facility Registration
If you do not have an SHI card:
You can purchase SI and register FV Hospital as your Primary Healthcare Facility at:

Social Insurance offices at the commune/ward/town level.

Vietnam Social Security (VSS) collection agents (e.g., Post Office, People’s Committee, or authorized organizations).

Online via the National Public Service Portal.
If you already have an SHI card (registered elsewhere):
You can transfer your registration to FV by:

Completing the process online via the National Public Service Portal.

Contact your original insurance purchase location for guidance.

Visiting a local Social Security office/agent, bringing your SHI Card, Citizen ID (CCCD), and Form TK01-TS.
