How To Register FV Hospital As Your Primary Care Facility
If you do not yet have a social health insurance card
You can purchase a social health insurance card and register FV Hospital as your primary care facility at:

Your local Social Insurance Office.

An authorised Social Insurance collection agent (such as a post office, People’s Committee or other authorised organisation).

The National Public Service Portal.
If you already have a social health insurance card and wish to change your primary care facility to FV
You can request a change of primary care facility through:

Using the VssID app or the National Public Service Portal.

Contacting the organisation through which your insurance was purchased (such as your employer, company or school).

Visiting your local Social Insurance Office or an authorised collection agent with your card, National ID or Level 2 VNeID, and form TK01-TS.
Note:
You may change your registered primary care facility during the first 15 days of each quarter (January, April, July and October).
